These are the new DOT Medical Examiner's Certificates (cards) with an expiration date of 11/30/2021,
to be completed by a certified medical examiner. The certificate (
6-5/8" X 4-3/4") easily
folds to the size of a credit card, so it fits conveniently into the driver's wallet. Each certificate is printed on 2-ply, carbonless paper, with one copy to the driver, one copy to the motor carrier/employer.
Each certificate comes with a free clear vinyl protective cover. This item contains 100 certificates and 100 covers.
